Effective January 1, 2004, the Prescription Drug Program provided through District Council Ironworkers Funds of Northern New Jersey Welfare Fund will be administered by National Medical Health Card Systems, Inc. (NMHC Rx). This program replaces the benefit through Express Scripts/CFI.
Your NMHC Rx Prescription I.D. Card entitles you and your eligible dependents to purchase medications from your local participating pharmacy or through our mail order vendor, RxDN in Philadelphia, PA. There are more than 56,000 pharmacies in the NMHC Rx network. Simply present your prescription and your new NMHC Rx Prescription I.D. Card to the pharmacist. The pharmacist will process the prescription through the NMHC Rx on-line claims system.
Under the plan, the following coinsurance rates will remain the same as of July 1, 2003:
• Retail prescriptions will continue to be filled at your local pharmacy and then submitted to the fund office for reimbursement. The fund office will reimburse the amount of each prescription with a co-pay charge of 10% with minimum of $5.00 and maximum of $50.00.
• Maintenance drugs submitted to RxDN mail order will be subject to a co-pay charge of 10% with minimum of $5.00 and maximum of $50.00.
• Retirees will still be subject to their annual $250.00 per member/per dependent deductible and then a co-pay charge of 10% with a minimum of $5.00 and maximum of $50.00.
You will receive a new prescription card in the mail prior to January 1, 2004 as well as information regarding the mail order program provided through RxDN and administered by NMHC Rx. Through this mail order program you can obtain your prescription medications and experience the convenience of home delivery without delay.
